· According to the World Health Organization, in 2018 there were 570000 cervical cancer cases around the world and 311000 women died from cervical cancer. Also, cervical cancer is the fourth most common cancer in women.

· Cervical cancer is the second most common cancer in women in Sri Lanka.  According to the World Health Organization, in 2018 there were an estimated 1136 cervical cancer cases in Sri Lanka and it is 8.8% of all female cancer patients.

What is cervical cancer?

The growth of the abnormal cells in the lining of the cervix, which is the lower part of the uterus that connects to the vagina is called cervical cancer.  There are mainly two types of cervical cancer,

1. Squamous cell carcinoma: Begins in the thin, flat cells lining of the outer part of the cervix which projects into the vagina.

2. Adenocarcinoma:  Begins in the column-shaped glandular cells that line the cervical canal.

70% of cervical cancers are squamous cell carcinoma type and adenocarcinoma is less common and it is more difficult to diagnose.

Causes of cervical cancer

  1. Human papillomavirus: Infection with the common human papillomavirus is a cause of approximately 90% of all cervical cancers.

2.  Sexual history: If the woman has had multiple sexual partners, began having sexual relations before the age of 18, and has a partner who has had sexual contact with a woman with cervical cancer the risk of developing cervical cancer is averagely high.

Risk factors of cervical cancer

· Smoking

· Weakened immune system

· Several pregnancies

· Giving birth at a very young age

· Long term use of the contraceptive pill

· Family history

Symptoms of cervical cancer

Most women do not have any signs or symptoms of a pre-cancer. Symptoms do not appear in the early stage of cervical cancer. However, the following symptoms can appear over time.

· Blood spots or light bleeding between or following periods.

· Menstrual bleeding is longer and heavier than usual.

· Bleeding after intercourse, douching or a pelvic examinations.

· Increased vaginal discharge.

· Pain during sexual intercourse.

· Bleeding after menopause.

· Unexplained, persistent pelvic.

Treatment for cervical cancer

· Surgery – Removing the cervical cancer tumor.

· Radiation therapy – Uses high energy rays to kill cancer cells.

· Chemotherapy – Uses a combination of drugs to destroy cancer cells or slow down the growth of the cancer cells.

The choice of treatment depends mainly on the size of the tumor and whether the cancer has spread.

· ලෝක සෞඛ්‍ය සංවිධානයට අනුව, 2018 දී ලොව පුරා ගැබ්ගෙල පිළිකා රෝගීන් 570000 ක් වාර්තා වූ අතර කාන්තාවන් 311000 ක් ගැබ්ගෙල පිළිකා නිසා මිය ගියහ. එසේම ගැබ්ගෙල පිළිකා කාන්තාවන් අතර හතර වනුව බහුලව දක්නට ලැබෙන පිළිකාව වේ.

· ගැබ්ගෙල පිළිකා යනු ශ්‍රී ලංකාවේ කාන්තාවන් අතර බහුලව දක්නට ලැබෙන දෙවන පිළිකාවයි. ලෝක සෞඛ්‍ය සංවිධානයට අනුව, 2018 දී ශ්‍රී ලංකාවේ ගැබ්ගෙල පිළිකා රෝගීන් 1136 ක් ඇස්තමේන්තු කර ඇති අතර එය කාන්තා පිළිකා රෝගීන්ගෙන් 8.8% කි.

ගැබ්ගෙල පිළිකා යනු කුමක්ද?

යෝනි මාර්ගයට සම්බන්ධ වන ගර්භාෂයේ පහළ කොටස වන ගැබ්ගෙලෙහි ඇති අසාමාන්‍ය සෛලවල වර්ධනය ගැබ්ගෙල පිළිකා ලෙස හැඳින්වේ.

ගැබ්ගෙල පිළිකා ප්‍රධාන වශයෙන් වර්ග දෙකක් තිබේ,

1. චතුරස්රාකාර සෛල පිළිකා: යෝනි මාර්ගයට ප්‍රක්ෂේපණය වන ගැබ්ගෙලෙහි පිටත කොටසෙහි තුනී, පැතලි සෛල වලින් ආරම්භ වේ.

2. ඇඩිනොකාර්සිනෝමා: ගැබ්ගෙල ඇල රේඛා තීරු හැඩැති ග්‍රන්ථි සෛල වලින් ආරම්භ වේ.

ගැබ්ගෙල පිළිකා වලින් 70% ක්ම චතුරස්රාකාර සෛල පිළිකා වර්ගය වන අතර ඇඩිනොකාර්සිනෝමා දුලභ වන අතර රෝග විනිශ්චය කිරීම වඩාත් අපහසු වේ.

ගැබ්ගෙල පිළිකා ඇතිවීමට හේතු

1. මානව පැපිලෝමා වයිරසය: පොදු මානව පැපිලෝමා වයිරසය ආසාදනය වීම ගැබ්ගෙල පිළිකා වලින් 90% ක් පමණ වේ.

2. ලිංගික ඉතිහාසය: කාන්තාවට බහු ලිංගික සහකරුවන් සිටී නම්, වයස අවුරුදු 18 ට පෙර ලිංගික සම්බන්ධතා පැවැත්වීම ආරම්භ කර තිබේ නම් සහ ගැබ්ගෙල පිළිකා ඇති කාන්තාවක් සමඟ ලිංගික සම්බන්ධකම් පැවැත්වූ හවුල්කරුවෙකු සමග සිටීම, ගැබ්ගෙල පිළිකා ඇතිවීමේ අවදානම සාමාන්‍යයෙන් ඉහළ දමයි.

ගැබ්ගෙල පිළිකා ඇතිවීමේ අවදානම් සාධක

· දුම්පානය කිරීම.

· දුර්වල ප්‍රතිශක්තිකරණ පද්ධතිය.

· ගැබ් ගැනීම් කිහිපයක්.

· ඉතා කුඩා අවධියේදී උපත ලබා දීම.

· පවුලේ ඉතිහාසය.

ගැබ්ගෙල පිළිකා රෝග ලක්ෂණ

බොහෝ කාන්තාවන්ට පූර්ව පිළිකාවක් පිළිබඳ රෝග ලක්ෂණ නොමැත. ගැබ්ගෙල පිළිකා වල මුල් අවධියේදී රෝග ලක්ෂණ නොපෙනේ. කෙසේ වෙතත්, පහත දැක්වෙන රෝග ලක්ෂණ කාලයත් සමඟ දිස්විය හැකිය.

· රුධිර ලප හෝ සැහැල්ලු ලේ ගැලීම් කාල පරිච්ඡේදයන් අතර හෝ පසුව.

· ඔසප් රුධිර වහනය වෙනදාට වඩා දිගු හා බරයි.

· සංසර්ගය, හෝ ශ්‍රෝණි පරීක්ෂණයෙන් පසු ලේ ගැලීම.

· යෝනි ශ්‍රාවය වැඩි වීම.

· ලිංගික සංසර්ගය අතරතුර වේදනාව.

· ඔසප් වීමෙන් පසු ලේ ගැලීම.

ගැබ්ගෙල පිළිකා සඳහා ප්රතිකාර

· සැත්කම් - ගැබ්ගෙල පිළිකා ගෙඩිය ඉවත් කිරීම.

· විකිරණ චිකිත්සාව - පිළිකා සෛල විනාශ කිරීම සඳහා අධි ශක්ති කිරණ භාවිතා කරයි.

· රසායනික චිකිත්සාව - පිළිකා සෛල විනාශ කිරීමට හෝ පිළිකා සෛල වර්ධනය මන්දගාමී කිරීමට ඖෂධ සංයෝගයක් භාවිතා කරයි.

ප්‍රතිකාර තෝරා ගැනීම ප්‍රධාන වශයෙන් රඳා පවතින්නේ ගෙඩියේ ප්‍රමාණය හා පිළිකාව පැතිරී ඇත්ද යන්න මතය.
